General Guidelines
Note: To initiate your claim, your policy must be fully paid, with no outstanding premium balance.
We usually require the following documents to process your claim. It's helpful if you can send all your documents together, as this makes it easier and quicker to review your claim. Notification can be made through phone call (028 236-6481) or via email at claims@moneymax.ph.
- Police Report/Incident Report or Notarized Affidavit of Accident (This should indicate the Date and Time of the incident, Where and How the Incident happened and Point of impacts/Damaged portion)
- Clear and updated copies of your OR and CR
- Clear copy of Driver’s License with OR
- Repair estimate from an accredited shop (please see attachment for the complete list)
- Vehicle Pictures (showing damaged portions, whole vehicle on the damaged side, and one showing your vehicle with plate number). Please refer to the guidelines here and here.
- Contact number and schedule for actual inspection
- Copy of your previous policy
Once all required documents are submitted, your insurance provider will proceed with processing your request. Further investigation may be conducted if deemed necessary.

IMPORTANT REMINDERS:
- We reserve all the rights of the insurance provider to request any additional documents necessary for our request.
- Premium payment should be paid in full before processing the claim request.
- The acknowledgment of the claim request and any documentation are not an acceptance of liability.
- Approval of claim will be reviewed and evaluated by our mutual Insurance Provider.
- Repair of the vehicle should start after the issuance of approval.
- No acceptance of Amicable settlement/any amount from the adverse party.
- The insurance provider will only start the review and evaluation of your claim upon the completion of claim requirements.
